Application or renewal of authorization for therapeutic use (TUE) of doping substances (Form)

French Anti-Doping Agency (AFLD)

Allows you to apply for authorization to use products on the list of prohibited doping substances for therapeutic purposes. The same form must be used if you want to renew an authorization.

This form must be sent to the French Anti-Doping Agency (AFLD) with a €30 to the order of 

Accounting Officer of AFLD

, by registered letter with acknowledgement of receipt.
