Mid-term review of the Ad'AP scheme (Form)

The mid-term review, provided for in the Ad'AP mechanism, takes the form of an online questionnaire. At the end of this questionnaire, you will be asked to validate and print your answers. The paper document will consist of the elements you have filled in. You will have to date and sign it before sending it, accompanied by the necessary attachments, to the DDT(M) or the service of the Paris police department, in charge of the follow-up of your Ad'AP.

Once the questionnaire is completed, once you press the "Send" button, you will not be able to go back to view or edit your answers. A PDF that you need to save will be generated.
