Health Supplements (CMU-C and ACS): you can apply online

This requires:

• enter your Family Allowance Fund recipient number;
• confirm or modify the composition of its focus;
• provide the necessary supporting documents in electronic format: tax or tax situation notice (Asdir), or property tax notice, or local housing tax notice, or tax and social situation statement of the country(ies) concerned in case of residence abroad in the last 12 months, and for the applicant for active solidarity income (RSA), the attestation of presumed resources below the active solidarity income lump sum (RSA) issued by the FAC or the MSA;
• choose the managing body of the CMU-C;
• validate the form.

You will receive an electronic acknowledgement of receipt in the email of your Ameli account. The application is then sent to the Primary Health Insurance Fund (CPAM) of your residence (or home) which has a period of 2 months to study the application and inform you of its decision. The Letter of Rights is made available in your Ameli account after the validation of the file and sent in paper format to your mailing address.

Online application is not possible when:

• the insured person is a minor;
• the insured person has closed rights to health insurance (in the case of expatriation, for example);
• the insured uses several mutual societies;
• the insured person does not have an assessment notice;
• the household is composed of more than 5 persons;
• the insured has already made a similar request less than 21 days ago;
• the mailing address entered in the ameli account is incorrect or incomplete (the insured can then change his address from the account. Once the change request has been registered by their primary cash register, they will be able to reapply for CMU-C or ACS online on their friendly account).

Individuals must submit their application using the paper form that can be downloaded to the Ameli account and send it back with supporting documents to their Primary Fund.
