Application - Judicial declaration of death (after disappearance) (Form 15456*02)

Allows you to request a judicial declaration of death.

To be presented to the public prosecutor of the court of justice where a relative has disappeared in circumstances likely to cast doubt on his or her survival.

You must submit the request or send it (by mail) RAR: titleContent(b) the prosecutor of the court at any of the following places:

  • Place of death or disappearance if it occurred on French territory
  • Home or last residence of the disappeared person if the disappearance took place abroad
  • The home port of the aircraft or vessel that carried the person in the event of an air or marine disaster
  • Paris, in the absence of any other place
