Claim for maternity leave (Form 15411*01)

Form allowing the father of a child, in case of death of the mother after birth, to apply for postnatal leave for the remaining period.

Paternity leave is postponed until the end of postnatal leave.

If the father of the child does not apply for postnatal leave for the remainder of the period, such leave may be granted to the person who lived in a couple with the mother, if she asks.
