Application form for disability pension for civil servant, magistrate or military member:

• Disabled, whether or not attributable to service, during employment in the public service, and no longer allowing him to work
• Or suffering from a disability which no longer allows him to work, which occurred while he was no longer performing his duties in the public service (during an availability, after resignation, etc.) and which justifies at least 15 years of service in the public service
• Or who has at least 15 years of service in the public service and whose spouse has a disability that does not allow him or her to work.
