prepared by the Pension Insurance, the National Solidarity Fund for Autonomy (CNSA) and the Mutual Social Agricultural Organization (MSA). He's been deployed since the 1ster October 2023 in Metropolitan France and Overseas.

What the form proposes

You can, using this unique form, request one of these 2 services depending on the level of loss of autonomy:

  • theHome support for older people pension funds: this is aid from the Pension Insurance for retirees from the general scheme or the State civil service, and from the Agricultural Social Mutual Society (MSA) for retired farmers;
  • theCustom Autonomy Allocation (APA), proposed by the departments.

A questionnaire integrated into the form allows to determine the level of autonomy of the person and to identify the most suitable help for his or her needs.

How do I apply for assisted living at home?

Depending on the department, the aid can be applied for:

  • either by the new Cerfa form, to be completed and sent by post to your department's independent service or to the pension fund;
  • either by a on-line service via FranceConnect.

Use the simulator to find out what approach is in effect in your department.
